One form for every program area — club teams, camps, clinics, private lessons, leagues, travel events, tournaments, equipment support, and emergency hardship requests. Applications are reviewed monthly and treated confidentially.

Proof of total household income is required (e.g. CRA Notice of Assessment, proof of social assistance, or foster parent status). PDF or photo, up to 5 files, 10 MB each. Documents are stored privately and only visible to the review committee.
